MISSION FOR SAFETY

The mission of the Division of Agriculture’s Environmental Health and Safety Department is to promote a safe and healthy working environment for all UADA employees. We will:

  • Support the involvement of all employees and students in the success of our health and safety program.
  • Promote health and safety as a part of every location and work site in order to enhance knowledge of safe and healthy practices.
  • Provide access and advice on health and safety regulations, procedures and standards and helping ensure continued compliance.
  • Set a good example by practicing and promoting safe behaviors.

Hazardous Waste Pickup Request

Please complete the Hazardous Waste Pickup Request Form and submit it to the Environmental Health and Farm/Lab Safety Director, Christine Bryant, at cb130@uark.edu.

Equipment Clearance Form

If you need to remove laboratory equipment from service for maintenance, relocation, transportation for storage, or surplus/disposal, complete the Equipment Clearance Form and attach it to the equipment before such action is taken.

Emergency Eye and Face Wash and Shower Forms

Training Log

All laboratory personnel must be trained in the proper testing and maintenance required for emergency eye/face wash and shower equipment. Find the requirements for training and maintain a log of training activities using the downloadable Emergency Eye/Face Wash and Safety Shower Training Log. This log must be maintained along with unit inspection records.

Weekly Inspection Checklists and Logs

Emergency Eye/Face Wash and Safety Shower equipment must be inspected weekly. Please download the appropriate checklist and log and maintain the inspection records.

Lab Close-Out Forms

Close-Out Notification

When preparing to vacate a laboratory space for relocation, please start by completing the Lab Close-Out Notification Form as early as possible — preferably at least 3 months — prior to the move.

Close-Out Certification

Prior to the close-out survey, please thoroughly review the Lab Close-Out Certification Form to certify that potentially hazardous materials have been properly accounted for and transported and/or disposed.
